Background:  Fungal diseases are mostly opportunistic infections with a great variety of common and uncommon pathogens. Up to 150 fungal species have been shown as potential human pathogens involving all body sites. Invasive mycoses are increasingly recognized as a primary cause of morbidity and mortality especially in immunocompromised patients. These infections are mainly caused by yeasts and moulds. The mortality rate of invasive fungal infections is 40-100%. A rapid diagnosis improves the outcome. Non-invasive fungal infections (such as infections of the urogenitary tract, of the eye, of the skin, etc.) are mainly caused by Candida, Aspergillus, Acremonium, Fusarium and dermatophytes. Concerning bacterial infections, broad‐range PCR targeting the 16S rDNA gene is useful when antimicrobial therapy has already been initiated or when culture remains negative due to infections caused by bacteria with unusual growth requirements.

Description:  PanReal Kit Fungi & Bacteria is a real-time PCR for the qualitative detection of the of fungi and bacteria in samples purified from bronchoalveolar lavages (BAL), aspirates, cerebrospinal fluid, tissue, paraffin embedded tissue and fungal/bacterial colonies. Due to contamination, PanReal Kit Fungi & Bacteria is less sensitive compared to species-specific assays. The kit is suitable only to a limited extent for the detection of fungi in blood samples. For phylogenetic identification of the pathogen, the PCR product has to be sequenced and analysed by BLAST. Our carefully designed primers and probe ensure highest sensitivity and specificity. The kit consists of an assay mix for the detection of the pathogen as well as a positive control and an internal positive control (IPC) system. To minimize PCR cross-contamination, the included reaction mix contains dUTP and uracil-N glycosylase (UNG).

PCR platforms: PanReal KitFungi & Bacteria was validated with the ABI® 7500 Fast Real-Time PCR System (fast cycle parameters are not supported, Thermo Fisher Scientific), and was also tested with Mx3005P® (Agilent), qTOWER3G (Analytik Jena), MIC instrument (bio molecular systems), LightCycler® 480 II (Roche), CFX Dx (Bio-Rad) and T-COR 8 (Tetracore) and cobas Z 480 Analyzer (Roche). It is also compatible with other real-time PCR instruments which detect and differentiate fluorescence in the FAM and Cy5 channel (e.g. QuantStudio 5 / 6 / 7 (Thermo Fisher Scientific)).

The kit is designed for broad-range molecular detection of fungal and bacterial DNA by targeting the ITS2 region of fungi and the V3 region of the bacterial 16S rDNA gene using multiplex real-time PCR.
Validated specimens include bronchoalveolar lavage, aspirates, cerebrospinal fluid, tissue, paraffin-embedded tissue, and fungal or bacterial colonies. The kit is suitable only to a limited extent for fungi in blood and is not suitable for bacterial detection in blood.
Fluorescence detection is channel-specific: fungi are detected in the FAM channel, bacteria in the VIC channel, and the internal control in the Cy5 channel.
PCR products can be sequenced and analyzed by BLAST against NCBI databases to identify the specific fungal or bacterial species present in the sample.
While broad-range PCR can reveal pathogens that are difficult to culture, it is prone to contamination from environmental DNA, which may lead to false positives. Careful interpretation and sequencing confirmation are required.
